Independent Contractor/Host Home Provider Requirements

In order for your application to become a provider to be considered, you must meet at least the following criteria:

• Able to pass a Criminal Records check
• 21 or over
• Live in a home that meets HUD, State and DDRC/QLO standards
• Have the financial resources required to open and operate the home
• Successfully complete the Basic Training Course and first aid/CPR
• Maintain professional liability insurance and automobile liability insurance as required by law

• Understand what it means to be an Independent Contractor/Host Home Provider
• Understand and follow state regulations around providing services as an Independent Contractor/Host Home Provider
  → Visit CO Secretary of State
