Resource / Service Coordination

DDRC Resource Coordination/Service Coordination (also referred to as case management) provides person-centered service coordination for children and adults with intellectual and developmental disabilities or developmental delays in Jefferson, Clear Creek, Gilpin and Summit counties in Colorado.

Resource Coordination provides determination of eligibility for intellectual and developmental disability services, referral and assessment, service plan development and service coordination for individuals enrolled in services for persons with intellectual and developmental disabilities. Your resource coordinator or service coordinator will help you understand what services are available to you, the enrollment process, service and support options and service agency and provider options. Resource/service coordinators develop a service plan with you to help meet your needs and goals and ensure that available funding and services are delivered effectively and to your satisfaction.

Colorado refers to this service as service coordination for children ages 0-3.

DDRC primarily uses the terms resource coordination rather than case management due to feedback that it is a more person centered and dignified term, however, the Center for Medicaid/Medicare Service (CMS) uses the term case management so you will hear these terms interchangeably.
